In order to install Curt trailer hitch # C15800 on your 2012 Dodge Ram 4500 cab and chassis model you will have to drill into the frame because, like you have said, there are no holes in the frame already. The rivet that is in the rear most part of the frame also will come out and get replaced by a bolt.
Installation is very similar to hitch # C15400 (just the capacities are different) and I have linked the instructions for it for you to view. The hitches install with 3 bolts on each frame rail. Drilling is required for all of the hitches that fit your truck.
