I have a 2012, but it shouldnt make a difference. -Previously, I converted to LED taillights and added load resistors also. Will that be an issue?

Yes, the Curt T-Connector Vehicle Wiring Harness with 4-Pole Flat Trailer Connector # C56333 will work on your 2012 Jeep Patriot. It uses a powered convertor so the power pulled from your factory harness will be very limited.
This system is designed to plug into the factory taillight connectors so as long as those were not changed with the taillights than the kit will remain plug and play at the taillights.
I recommend using the Mounting Brackets # 18144 and 18140 to secure the 4-way to your Jeep. This will protect the wiring from getting caught on road debris while you drive and gives you a solid connection without needing to drill into your vehicle.
