Expert Reply:
For the 2019 Toyota Highlander with a full sized spare tire I recommend the etrailer Trailer Hitch Receiver - Custom Fit - Matte Black Finish - Class III - 2" # e98844. This hitch is a confirmed fit for your full-sized spare, since you don't have the aluminum rim. I have attached an installation video for your reference.
I recommend Tekonsha T-One Vehicle Wiring Harness with 4-Pole Flat Trailer Connector part # 118449 for the wiring. This particular harness includes the converter since your vehicle does not have a tow package. The Tekonsha brand wiring has proven to be more reliable and durable than the other brands especially the converters. I have attached an installation video for your reference.
If you plan on doing some towing you may consider the etrailer ball mount kit # 989900. It comes with 2 ball mounts; one with a 3/4 inch rise or 2 inch drop and one with a 2-3/4 inch rise or 4 inch drop. It also includes a 1-7/8 inch ball, 2 inch ball, hitch pin with clip, and a storage bag. If you are not sure how much rise/drop you may need in a ball mount to tow your trailer level, I recommend to take a look at the helpful article I have attached about how to figure the amount of rise or drop you may need.
