If you have confirmed that the fully loaded and ready to tow tongue weight of the trailer in question is a maximum of 500 lbs, I would recommend the Reese Trunnion Bar Weight Distribution System, part # RP66540. Be sure that anything stowed in the bed of the truck behind the rear axle is added into that tongue weight figure.
This system is recommended for trailers with tongue weights ranging from 200-600 lbs, and should work perfectly for your 5000 lb trailer. The weight distributing shank is included, but the appropriately sized trailer ball with 1-1/4 inch diameter threaded shank will be needed. For a 2 inch ball, use part # A-90, or part # 19286 for a 2-5/16 ball.
You will also need to confirm that your existing receiver hitch is rated for use with a weight distribution system.
