Yes, you will need an aftermarket trailer brake controller to equip your Audi Q7 for towing a trailer with brakes. The actual components included in an OEM "tow package" will vary by brand and model but the trailer brake controller itself is not included among them.
If your Q7 has a factory 7-way connector similar to the one shown in the linked photo you have the option for an exceptionally easy brake controller installation.
