Any reason to believe that a manual transmission will make this one harder to install?

For your 2018 Honda Accord the Curt hitch part # C11525 is a confirmed fit as long as your Accord is not the hybrid model. The fact that it's a manual transmission would not add any difficulty to the install process.
For tralier wiring you'd then want the part # 119250KIT, for a ball mount part # 3593, hitch ball # 19258 for a 2 inch ball and part # 19256 for a 1-7/8 inch ball.
