The Curt Gooseneck Flat Plate part # C65500 is designed to be installed with the Curt Gooseneck Rails part # C61502 that you referenced and is a universal fit rail that would require a bit of fabrication to install on a truck like yours. To install this kit on your truck frame you are going to need to drill some holes in the frame.
By fabrication the cross rails need to be shortened 6-7 inches as well as drilling through the frame if you were to install this in a 2000 Ford F-350 cab and chassis.
