I have a solution for your clearance issue, but the Lippert Jack # LC285318 cannot be rotated. You can get a bit more clearance by using a longer ball mount like this extra long ball mount # D-26. It measures 12-5/8 inches from the hitch pin hole to the ball hole. This will give you more clearance. There are two ways of raising the height of your fifth wheel. You can use taller spring hangers than what you currently have. Keep in mind that due to the movement allowed by the shackle links as well as the fact that a spring will flatten when compressed, using a 3 inch taller spring hanger/equalizer would not gain you a full 3 inches of increased height.

The receiver tube isn't always completely level due to manufacturer issues and with how it could sit when bolted to the frame of the vehicle. It's great that you are being cautious but so long as the tow rating of your 2012 Chevy Silverado 2500 and weight capacity of your fifth wheel hitch are both higher than the GVWR of your fifth wheel trailer than you are safe to tow.
In regards to the fifth wheel hitch, I highly recommend a 20,000 lb capacity hitch to ensure a high enough capacity; that may seem like overkill but hitches generally start at a 16,000 lb capacity with the next highest rating being 20,000 lbs.
